I hereby acknowledge that I have voluntarily enrolled in an online educational program provided by Fulcrum Aesthetics and Crux Academy. By purchasing this video, I understand and agree to release Fulcrum Aesthetics, and Crux Academy, their employees, agents, and representatives from any and all claims, liabilities, demands, damages, or causes of action that may arise out of or be related to my participation in the online educational program.I understand and acknowledge that there are certain risks and hazards associated with online education, including but not limited to technical issues, internet connectivity problems, and other unforeseen events. I acknowledge that I am participating in the online educational program at my own risk and that Fulcrum Aesthetics and Crux Academy shall not be held liable for any such risks or hazards.I further understand and agree that I will not be entitled to any refunds for the online educational program, regardless of whether I complete the program or not.I attest that I am in good standing with my licensing board and am a licensed medical provider that is legally able to perform the skills being taught in the online educational program.I have read this release form and fully understand its contents. I hereby release and discharge Fulcrum Aesthetics and Crux Academy from any and all claims, liabilities, demands, damages, or causes of action, and agree to indemnify and hold them harmless from and against any and all claims or liabilities arising out of or in connection with my participation in the online educational program.
